Nurse Case Manager RN

RN Case Manager 

Ideal Hours 10am-6pm

On Call Requirement Every 7th Weekend

The Case Manager serves as a member of the interdisciplinary team, assumes planning, responsibility, and
accountability for resident care of a designated unit, in conjunction with the Unit Manager (UM) and/or Assistant
Director of Nursing Services (ADNS), and in accordance with Federal and State regulations and company/facility
policies, procedures and care plans.

Requirements:


* Current and valid Registered Nurse license in the state of Indiana.


* Proficient medication management skills.


* Ability to conduct thorough assessments and accurately document changes in resident conditions.


* Strong passion for geriatric nursing and commitment to senior care excellence.
