Position Summary

 Under the direct supervision of the Prevention Supervisor, the Caseworker is primarily responsible for providing case management and counseling services to identified youth and families.  Responsibilities include advocacy, referral and linkage, group, individual and family counseling and crisis intervention.  

Essential Job Functions



* Utilizing the task centered case management model, implements and coordinates services to children and their family's having trouble in the home and/or in school, aimed at preventing placement of children outside the home and/or at improving academic performance.  


* Provides intervention and support to families in crisis intended to keep the family unit intact.


* Completes all required documentation, in timelines in accordance with program standards.

Duties and Responsibilities



* Assesses children and families for risk, develops and implements service plans to address risk factors, drafts petitions and testifies in Family Court as necessary.  Provides Foster Care Services and locates placements when needed.



* Advocates on behalf of children and their families to ensure adequacy of services (i.e., educational, legal and community) by providing linkage and coordination with the county providers and agencies.  



* Processes and reviews intake of all referrals to program.  



* Provides services in a manner which is culturally competent.
