LVN Noc Shift Full-Time

Licensed Vocational Nurse (LVN) - Full-Time NOC Shift

Kern River Transitional Care Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the Full-Time NOC Shift .

We are looking for a dedicated nurse who is committed to providing excellent resident care and working collaboratively with our interdisciplinary team.
Compensation

Pay Range: $28.00-$32.00 per hour, based on experience
NOC Shift Differential: Additional $1.00 per hour
Essential Duties and Responsibilities


* Provide direct nursing care to residents in accordance with physician orders, resident care plans, and facility policies.


* Administer medications and treatments accurately and timely.


* Monitor residents for changes in condition and promptly report concerns to the appropriate physician or nursing supervisor.


* Complete accurate and timely nursing documentation.


* Participate in the development and implementation of individualized resident care plans.


* Communicate effectively with residents, families, physicians, nursing staff, and members of the interdisciplinary team.


* Assist with admissions, transfers, and discharges as needed.


* Obtain and document vital signs and other clinical information.


* Ensure physician orders are carried out accurately.


* Supervise and provide direction to CNAs as assigned.


* Follow infection prevention and control procedures.


* Respond appropriately to resident emergencies and changes in condition.


* Maintain confidentiality of all resident and facility information.


* Ensure residents are treated with dignity, respect, and compassion.


* Follow all federal, state, and facility nursing regulations, policies, and procedures.


* Perform additional nursing duties as assigned.

Qualifications


* Current and active California LVN license in good standing.


* Current CPR/BLS certification as required.


* Previous skilled nursing, long-term care, rehabilitation, or post-acute experience preferred.


* Strong clinical assessment and documentation skills.


* Excellent communication and organizational abilities.


* Ability to prioritize resident needs and manage multiple responsibilities.


* Ability to work effectively with residents, families, nursing staff, and other healthcare professionals.
