Summary: The Therapist provides integrated behavioral health services in the community of their assigned regions.

This role delivers assessments, brief interventions, individual and family therapy, crisis evaluations, and collaborative care coordination for children, youth, adults, and families.

The Therapist implements a trauma informed model while working closely with medical providers, school systems, and community partners to ensure strength-based care with wraparound support.

Essential Duties & Responsibilities: Responsible for fulfilling all duties related to emergency preparedness, evacuation, and client safety, including support of individuals who may not be capable of self-preservation, in accordance with TN Voices Employee Policies and TN State Department regulations and CARF Standards.

Provides individual therapy to children, adolescents as identified and clinically determined.

Diagnostic Interview and assessment to determine eligibility for services.

Complete Individual Treatment Plans identifying goals for services with the identified client and guardian for minor per policy in the EHR.

Completes measurement-based screenings and provides evidence-based and evidence-informed treatment modalities.

Documents progress notes for all therapy sessions per policy in agency EHR.

Collaborates as needed with other resources within TNV and community.

Complete transition and discharge plan and summary.

Provide necessary treatment summaries and documentation as requested.

Ensures treatment services conform to best-practice standard of care.

Works as a team with the Family Support Specialist to ensure coordinated service delivery and at least weekly to twice a week face-to-face contact with family/youth.

Attends court/CFTMs and communicates with Community partners regarding youth/families' participation in services and progress in completing goals established to address mental health needs.

Assist FSS in identifying community resources and helping families engage with resources that will sustain family progress beyond discharge from program.

Available during work hours for crisis support and assist in creating a crisis plan for 24/7 support.

Availability to work outside of the general operating hours to meet the needs of the family/youth.

Documents progress, safety plan, treatment plan, etc.
