Family Support Specialist
Working independently and with minimal supervision, conducts home visits with assigned families to provide support services to improve child health, growth and development outcomes.
Facilitates parents in achieving improved problem solving, goal setting, and advocacy skills.
Conducts professional assessment, planning and evaluation in developing and implementing supportive services, interventions, and referrals.
Collaborates and consults with multidisciplinary and multi-agency teams to achieve desirable outcomes for families and communities.
Essential Duties Initiates and maintains regular and long-term contact and support with families.Conducts home visits with assigned families to provide support services consistent with program utilizing a family-centered, strength-based model.Assists parents in improving skills to optimize home environment and strengthen parent-child relationship to improve child health, growth and development outcomes.Facilitates increased abilities and success of parents in problem solving, improving family support system, serving as family advocate, and establishing goals and effective plans to achieve them.Establishes professional, trusting relationships with families to facilitate effective provision of services while maintaining appropriate personal and programmatic boundaries.Utilizes professional training and judgment in assessing, planning, implementing and evaluating services and supports provided on an ongoing basis.Documents services, plans, and outcomes utilizing established forms and electronic health records, in accordance with established standards for timeliness, accuracy, and content.Collaborates with healthcare providers, community partners, and other agency staff in identifying and assessing high-risk families and coordinating interventions or referrals.Ensures accessibility by families after regular work hours for emergencies.
Other Duties & Responsibilities Participates in Emergency Preparedness Incident Command System (ICS) as assigned.Performs other duties and responsibilities as necessary in response to emergencies, workloads or staffing shortages.
Works flexible hours as needed.Supports other program and office functions as necessary to ensure effective customer and community service.Maintains up-to-date vaccines including flu (influenza,) in accordance with CDC guidelines: Recommended Vaccines for Healthcare Workers.
