Housing Support Mental Health Practitioner

Mental Health Practitioner (MHP) Fernbrook Family Center This position is 0.5 FTE working in supplemental housing with adult clients and 0.5 FTE working in home and community settings with clients ages 10 and under.

Job Summary Fernbrook Family Center is seeking a compassionate and motivated Mental Health Practitioner (MHP) to provide community-based mental health services to children, adults, and families in supplemental housing and home and community settings.

Working under the supervision of a Qualified Mental Health Professional (QMHP), MHPs support clients in building skills that enhance daily functioning, independence, and overall well-being.

Services are delivered in a variety of settings, including client homes and community environments.

Key Responsibilities Provide skills-based mental health services through individual, family, and group interventions Meet productivity benchmarks and weekly billable service expectations Use culturally responsive and evidence-informed approaches to support client needs Complete accurate and timely clinical documentation Collaborate with Fernbrook staff and external service providers Participate in supervision, staff meetings, and ongoing professional development Complete 30 hours of mental health-related continuing education every two years Uphold Fernbrook's mission, values, and policies Perform additional duties as assigned Required Skills & Abilities Strong ethical decision-making and professional boundaries Excellent time management and organizational skills Effective written and verbal communication Ability to work both independently and as part of a team Openness to feedback and professional growth Basic computer proficiency (or willingness to learn) Commitment to culturally responsive, client-centered care Education & Experience Requirements Must meet Minnesota Statute requirements for: Mental Health Practitioner OR Clinical Trainee Additional Requirements: Valid driver's license, reliable transportation, and auto insurance Acceptable driving record Ability to pass a DHS background check Preferred Qualifications Experience working with children, adults, and families Bilingual or multilingual abilities Work Environment & Schedule Flexible schedule, typically between 7:00 a.m.
