LVN Case Manager
Precision Spine Care is seeking a dedicated and compassionate LVN Case Manager to join our team in Longview, Texas.
This position combines hands-on patient care with clinical coordination responsibilities to ensure patients receive exceptional care throughout their treatment journey.
The ideal candidate is a licensed vocational nurse who excels in patient communication, care coordination, and team leadership within a fast-paced medical environment.
Key Responsibilities Obtain, verify, and document health histories for new and established patients.
Perform patient assessments and gather clinical data, including biological specimens for laboratory testing.
Provide direct patient care and administer medications and treatments as directed by providers.
Assist physicians with examinations, diagnostic procedures, treatments, and minor medical procedures.
Accurately document assessments, interventions, medications, test results, and patient interactions in the electronic health record (EHR).
Enter physician orders for Urine Drug Screens and ensure results are routed appropriately for review.
Coordinate laboratory, radiology, and prescription-related activities.
Provide patient education and communicate physician instructions to promote wellness and treatment compliance.
Respond to patient phone calls regarding clinical concerns, treatment questions, and postoperative care.
Oversee physician scheduling, including time-off requests and schedule modifications.
Support daily clinic operations by maintaining workflow efficiency and ensuring accountability among clinical support staff.
Collaborate closely with physicians, clinic leadership, and operations teams to deliver high-quality patient care.
Qualifications Current, unrestricted LVN license in the State of Texas.
Current CPR certification.
Experience in a clinical or specialty care setting preferred.
Prior experience with NextGen electronic health records is a plus.
Strong computer skills, including proficiency in Microsoft Office applications.
Excellent verbal and written communication skills.
Strong organizational, interpersonal, and multitasking abilities.
Ability to work effectively under pressure while maintaining professionalism and quality patient care.
