Summary:TheCare Coordinatorprovidesstrength-based,client-centeredcoordination of servicesfor children, youth, adults, and families with behavioral health needs, justice involvement, social service involvement, or complex mental health challenges.

This role ensures that services arecollaborative,family-drivenorclient-driven, and delivered with high fidelity to wraparound andsystem-of-careprinciples.

The Care Coordinatorfacilitatesteam processes, develops care plans, links clients tosupports, responds to crises, andmaintainshigh-qualitydocumentation.

Essential Duties & Responsibilities: Direct Support & Care Coordination Provide regularface-to-faceservices in homes, the community, or program settings.

Engage clients and families in astrength-based-planning process.

Link clients to community resources, clinical services, naturalsupports, andskill buildingopportunities.

Assistwith appointments, transportation, advocacy, and navigating service systems.

Team Facilitation Coordinate andfacilitateFamily TeamMeetings,andcase conferences.

Partner with Family Support Specialists, therapists, peer specialists, and other professionals to support wraparound fidelity.

Help clientsidentifysupportive individuals toparticipatein the care planning process.

Assessment, Planning & Documentation Complete required assessments (e.g., caregiver strain tools, behavioral health assessments).

Develop and update Individualized Service Plans, Plans of Care, Crisis Plans, and Safety Plans.

Maintainaccurate,timelydocumentation including notes, evaluations, service agreements, and termination summaries.

Crisis Response Provide crisis support, both telephonic and in-person, and collaborate with clinicians to ensure stabilization.

Ensurecrisis and safety plans are completed and followed.

Program Collaboration & Compliance Participate in supervision, team meetings, and case reviews.

Maintain adherence to agency policies, confidentiality laws (including HIPAA), and CARF or program accreditation standards.

Support community engagement efforts, represent the organization positively, andparticipatein meetings or workgroups asrequired.

Program SpecificResponsibilities (Assigned based on department) Adult Mental Health:Assistwith budgeting, vocational goals, substance use interventions, and coordination with clinical teams.
