St.
Francis Healthcare Center is seeking a dedicated, organized, and compassionate Admissions Coordinator to join our team on a full-time basis.
The Admissions Coordinator plays a key role in supporting the admissions process, coordinating referrals, communicating with residents and families, and working closely with clinical and administrative departments to ensure a smooth transition into our facility.

Key Responsibilities
* Coordinate and facilitate the admission process for new residents.
* Review and process referrals, medical records, and required admission documentation.
* Communicate with hospitals, physicians, case managers, residents, families, and referral sources.
* Obtain and verify insurance and demographic information.
* Coordinate admissions with nursing, social services, business office, and other departments.
* Ensure all required paperwork and documentation are completed accurately and timely.
* Maintain accurate admission records and referral information.
* Follow up with referral sources regarding pending and prospective admissions.
* Assist with coordinating transportation and admission arrangements when needed.
* Provide residents and families with information regarding facility services, policies, and procedures.
* Help ensure a welcoming and organized admission experience.
* Maintain confidentiality and comply with HIPAA and facility policies.
* Support census development and building relationships with referral partners.
* Perform other duties as assigned.
Qualifications
* Previous experience in healthcare admissions, skilled nursing, post-acute care, hospital case management, or a related healthcare setting preferred.
* Strong verbal and written communication skills.
* Excellent organizational and time-management abilities.
* Ability to multitask and prioritize in a fast-paced environment.
* Strong attention to detail and follow-through.
* Professional and compassionate approach when interacting with residents and families.
* Knowledge of insurance verification and healthcare documentation preferred.
* Proficiency with computers and electronic medical record systems preferred.
* Ability to work collaboratively with clinical and administrative teams.
