Physical Therapist - Home Care/Home Health (Part Time PRN)

RiverCare, Home Care division of Riverview Retirement Community is seeking a compassionate and skilled Physical Therapist to join our dedicated therapy team.

This position offers the opportunity to provide comprehensive care to a diverse patient population, through our Home Health department.

Physical Therapist Key Job Responsibilities: Conduct comprehensive evaluations to assess patients physical abilities and establish customized treatment goals.Design and deliver therapeutic interventions, including exercises, manual therapy, and aquatic therapy, to enhance strength, mobility, and independence.Educate patients and their families on home exercise programs, adaptive techniques, and safety measures.Collaborate with an interdisciplinary team to provide holistic, coordinated care.Accurately document evaluations, treatment progress, and outcomes in compliance with state and federal regulations.Stay informed about advancements in physical therapy practices and integrate them into care delivery.
