Nursing Services Supervisor
Clinical Operations The Nursing Services Supervisor is responsible for the day-to-day operations of their designated region to ensure agency goals and superior patient care is achieved.
- Manages and directs a team of clinicians including RN Case Manager's, RN's, LPN's, Social Work and Home Health Aide's to ensure a safe, effective, and appropriate home care service is provided - Functions as the primary resource to associate questions and concerns.
Provides guidance to clinicians regarding questions concerning proper patient care.
- Meets with clinicians regularly on an individual basis to provide guidance and information related to specific issues.
- Collaborates with the QA department and provides education to staff regarding OASIS corrections and chart revisions - Provides direction to teams to assure client needs are met and services are provided according to the agency policies and procedures - Educates clinicians on clinical services, policies, and procedures, as needed - Serves as the clinical expert on home health guidelines and Medicare regulations to accurately respond to associate concerns and questions.
- Ensures accuracy, completeness, and timeliness of clinical documentation in accordance with Agency policies and procedures, regulatory requirements, and industry standards.
- Performs weekly review of clinician's productivity and reports to Area Manager - Coordinates 24 hour on-call and acts as a back-up to provide patient care if needed - Reviews on-call logs and provides a summary of reasons for calls and resolutions - Leads all weekly case conferences to review admissions, discharges and re-certifications, potential LUPA's, patient outcomes, team overall SHP scoring and other patient specific needs.
Assures discharge planning is completed and required documentation is assigned and/or returned for upload to chart.
- Conducts ongoing chart/clinician review to include: o New admission review o Frequency orders o Scheduled supervisory visits o Wound care o Visit exceptions: overlapping visits and visits less than 15 minutes o Outstanding visits: past due visits o Mileage: review for excessive mileage - Conducts ongoing Nurse meetings to perform in-services and promote educational training to clinicians based on Education Plan and identified regional skills needs.
Personnel Management - Reviews employee timecards/payroll daily via the HRIS to monitor overtime, pay premiums, on-call, absenteeism, and ETO requests.
Follows agency policy regarding ETO requests to assure adequate patient coverage is met.
- Collaborates with human resources regarding policy/procedure violations or performance transgressions to deliver performance improvement plans and corrective actions - Assists in screening and interviewing of new Agency personnel and makes recommendations to Director, Clinical Operations and Human Resources.
