What You'll Do

As an LVN, you will play a vital role in providing exceptional nursing care while supporting residents, families, and fellow team members.

Your responsibilities include:


* Deliver direct nursing care in accordance with physician orders and individualized care plans


* Safely administer medications, treatments, and nursing procedures


* Supervise and support Certified Nursing Assistants (CNAs)


* Monitor resident conditions and promptly communicate changes to physicians and the interdisciplinary team


* Complete accurate, timely, and compliant clinical documentation


* Admit, transfer, and discharge residents as needed


* Participate in care planning, quality assurance, and interdisciplinary meetings


* Maintain infection prevention and safety standards


* Support resident rehabilitation and restorative nursing programs


* Foster a positive, respectful, and compassionate environment for residents and families


* Provide leadership, mentorship, and guidance to nursing assistants throughout your shift

Qualifications

We're looking for candidates who have:


* Current California LVN license in good standing


* Graduate of an accredited LVN program


* Current CPR Certification


* Skilled Nursing Facility or Post-Acute experience preferred


* Experience with PointClickCare (PCC) is a plus


* Excellent clinical judgment, communication, and organizational skills


* A compassionate, dependable, and team-oriented approach to resident care

Physical Requirements

This position requires standing, walking, lifting, pushing, pulling, and assisting residents throughout the workday.
