1:1 Direct Service Professional

Key Responsibilities / Expected Deliverables: Provide direct 1:1 support based on the individuals service plan, goals, and preferences.

Implement person-centered plans with a focus on independence and community inclusion.

Support daily activities including personal care, communication, and behavioral support as needed.

Facilitate participation in community-based and meaningful day activities.

Accurately document all services in accordance with OE standards and regulatory requirements.

Monitor and report progress toward goals.

Ensure health, safety, and well-being at all times.

Maintain professional boundaries and effective communication with all stakeholders.

Demonstrate flexibility to adapt to changing needs and environments.
