Coordinator for CLASS

Job Summary: The Coordinator oversees services for individuals in the program, ensuring compliance, quality care, and accurate documentation while supporting clients and care staff.

Key Responsibilities: Submit assessments and documentation to the appropriate state health and human services department.

Develop and maintain individual plans of care and habilitation plans.

Ensure services align with approved individual plans.

Monitor service providers for quality and compliance.

Coordinate with case management agencies.

Maintain service records and evaluation documents.

Ensure timely billing and documentation.

Qualifications: High school diploma or GED.

Minimum of one year of administrative or clerical experience (experience in home health preferred).

Strong communication and organizational skills.

Proficiency in Microsoft Word, Excel, Outlook, and web browsers.

CPR certification (required; may be obtained upon hire).

General Requirements: At least 18 years of age.

Valid Texas drivers license.

Clear criminal background check.

Spanish language skills are a plus.

Physical Requirements: Ability to travel independently to client homes.

Ability to lift up to 50 lbs.

and assist with transfers (Hoyer lift, positioning, ambulation).

Ability to sit or stand for extended periods and work on a computer.
