We are seeking a compassionate and skilled Occupational Therapist (OT) to join our Home Care team and help patients regain function, maximize independence, and improve their quality of life.

As an Occupational Therapist with Cornerstone VNA, you'll provide one-on-one care in the home setting, allowing you to develop meaningful relationships with patients while creating personalized treatment plans that make a lasting difference.

You'll work collaboratively with an experienced interdisciplinary team dedicated to helping patients achieve their goals and remain safely at home.Enjoy the flexibility and autonomy of your schedule, while alsocaring for patients in their homes while building lasting relationships and seeing the direct impact of your work.

What You'll Do Evaluate patients in their home environment and develop individualized treatment plans.

Assess functional abilities, cognitive status, home safety, mobility, strength, and activities of daily living.

Implement evidence-based occupational therapy interventions to improve independence and safety.

Educate patients and caregivers on adaptive equipment, compensatory strategies, fall prevention, and home modifications.

Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care.

Monitor patient progress and modify treatment plans as appropriate.

Complete timely and accurate documentation within the electronic medical record.

Perform therapy reassessments and outcome evaluations in accordance with regulatory requirements.

Supervise Occupational Therapy Assistants (COTAs) in accordance with state regulations and agency policy.

Participate in quality improvement initiatives and ongoing professional development.

What We're Looking For Graduate of an accredited Occupational Therapy program.

Current Occupational Therapist licensure in New Hampshire and Maine.

Minimum of one year of home health experience preferred.

Minimum of two years of therapy experience preferred.

Strong clinical assessment and treatment planning skills.

Excellent communication and patient education abilities.

Ability to work independently while collaborating effectively within a multidisciplinary team.

Reliable transportation and ability to travel throughout the service area.

Passion for helping patients remain safe, independent, and successful at home.
