Behavioral Health Care Coordinator

As a WCBH Care Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management for a caseload of clients receiving community-based and in-office skills-based therapy.

You'll assess client needs, coordinate care, and connect individuals with community resources, while supporting clinical processes associated with mental health treatment.

Essential Duties and Responsibilities: Help clients and families understand treatment options and make informed decisions through regular in-office and phone check-insWork with the treatment team to facilitate coordination of care and information sharingParticipate in developing and implementing treatment plans, case management, and other clinical documentationPromote effective use of clinical resources, community resources, and other social supportsAssist clients in accessing resources, including sharing information and applying for benefitsCoordinate and communicate internally and externally to ensure continuity of services
