Position Summary

Centerville Post Acute is seeking a compassionate, organized, and customer-focused Social Services Assistant / Admissions Assistant to join our interdisciplinary team.

This dual-role position supports residents and their families throughout the admission process while assisting the Social Services Department in promoting residents' emotional, psychosocial, and discharge planning needs.

The ideal candidate demonstrates excellent communication skills, professionalism, empathy, and the ability to work effectively in a fast-paced healthcare environment.
Essential Responsibilities
Admissions Assistant


* Welcome prospective residents and families, providing exceptional customer service throughout the admission process.


* Coordinate admissions with hospitals, physicians, case managers, referral sources, and insurance providers.


* Collect and verify admission documentation, insurance information, and required paperwork.


* Assist with pre-admission assessments and ensure all required documentation is completed accurately and timely.


* Conduct facility tours for prospective residents and families.


* Maintain communication with referral sources to support census growth.


* Enter resident information into electronic medical records and admission systems.


* Coordinate with nursing, therapy, dietary, and other departments to ensure smooth resident admissions.


* Maintain confidentiality in accordance with HIPAA regulations.

Social Services Assistant


* Assist the Social Services Director with resident assessments and psychosocial evaluations.


* Help residents and families adjust to the facility environment by providing information, support, and community resources.


* Assist with discharge planning and coordination of post-discharge services.


* Maintain accurate documentation in residents' medical records.


* Help coordinate resident and family meetings as needed.


* Support residents in identifying social, emotional, and financial resources.


* Assist with advance directives, resident rights education, and grievance procedures.


* Participate in interdisciplinary care plan meetings.


* Promote resident dignity, independence, and quality of life.

Qualifications


* High school diploma or GED required; Associate's or Bachelor's degree in Social Work, Human Services, Healthcare Administration, or a related field preferred.


* Previous experience in healthcare, long-term care, admissions, customer service, or social services preferred.


* Excellent interpersonal, communication, and organizational skills.


* Ability to maintain confidentiality and professionalism.


* Strong computer skills and experience with electronic medical records preferred.
