Intake LPN

Summary The Intake Licensed Practical Nurse (LPN) plays a vital role in supporting the home health intake process by clinically reviewing referrals, coordinating patient admissions, and ensuring all documentation meets regulatory and agency requirements.

This position serves as a liaison between referral sources, physician offices, clinicians, and internal departments to facilitate timely, compliant, and efficient patient admissions while promoting exceptional patient care.

Responsibilities: Review and clinically scrub all incoming referrals for completeness, appropriateness, and eligibility for home health services.

Evaluate referral documentation to ensure it meets Medicare, payer, and agency admission requirements.

Identify missing clinical documentation and coordinate with referral sources to obtain required information.

Prioritize referrals based on urgency and patient needs.

Review physician orders for accuracy and completeness prior to admission.

Review laboratory results and identify clinically significant findings that require follow-up.

Review medication lists and medication changes to ensure accurate reconciliation before the start of care.

Communicate clinical concerns to the Clinical Director or assigned clinician as appropriate.

Serve as the primary point of contact for physician offices regarding intake-related clinical questions.

Obtain signed physician orders, certifications, and required documentation.

Coordinate clarification of physician orders and medication changes when necessary.

Maintain professional relationships with referral sources and physician practices to promote efficient communication.

Coordinate and manage the issuance of Medicare and payer-required non-coverage notices, including Notices of Medicare Non-Coverage (NOMNC) and other applicable notifications.

Ensure all required notices are delivered within regulatory timeframes.

Maintain accurate documentation of notices within the electronic medical record.

Collaborate with the Supply Coordinator to ensure all required patient supplies, wound care items, durable medical equipment (DME), and specialty items are ordered and delivered prior to or immediately following admission.

Verify supply needs based on physician orders and clinical assessments.

Communicate supply issues or delays to the clinical team promptly.

Ensure all admission documentation is complete before assigning patients to clinicians.

Monitor referral progress from receipt through admission.

Other duties as assigned.

Qualification Education Graduate of an accredited Practical Nursing program.

Current Licensed Practical Nurse (LPN) license in the State of Michigan.

Experience Minimum of two (2) years of nursing experience preferred.

Home health experience is strongly preferred.

Previous intake, case management, or utilization review experience preferred.

Experience working with electronic medical records (EMR) required.
