Mobile Crisis Case Manager

We are looking for individuals who: Have a Bachelor's degree in human services arena (Psychology, or Social Work) requiredHave at least one year of professional-level experience in human relations or related field is preferredAre CompassionateEnjoy Helping OthersAre EmpatheticAre Caring The Role: The role of the Mobile Crisis Case Manager is to provide critical support to individuals facing mental health and related crisis in our community.

What you will do: Respond to mobile crisis dispatches and provide on-site de-escalation and suicide risk screening, safety planning, and resource linkageOffer empathetic listening and emotional support to individuals in distressPlans for and provides effective, understandable, and respectful service to both staff and consumers that are compatible with an individual's beliefsGenerates accurate and timely documentation of service provision, provides accurate and timely summaries of client progress and/or service activities, which results in high quality records meeting all applicable standards and proceduresFollow up with clients, make referrals, and collaborate with community partners and treatment providersMeets contractual obligations for the program that allows the program and organization to legally bill for servicesProvides linkage, monitoring/follow-up, referral, advocacy and crisis intervention and sufficient follow-up to ensure that clients are connected to needed services and supports bothPerforms other related duties as assigned Qualifications: Bachelor's degree in human services arena (Psychology, or Social Work) requiredAt least 1 year of professional-level experience in human relations or related field is preferred Employment Requirements: Completion of a pre-employment drug screenCompletion of a background checkSatisfactory Driving Record If you meet the above qualifications, this is the role for you!
