Pay Range: $24-$25.50/hour

The base hourly range for this position is $24-$25.50 per hour for employees hired in the Central Valley, CA.

The pay range provided in this job posting represents the typical range or starting rate of candidates hired within the Central Valley region.

Candidates hired in Southern California or Bay Area locations may have higher starting salaries due to cost of living and market data differences.

Position Summary

Under the direction of the ECM Core Program Manager, the Lead Care Manager (LCM) provides comprehensive, whole-person care management services to Medi-Cal members enrolled in the Enhanced Care Management (ECM) Program.

The LCM serves as the primary point of contact for assigned members and their support systems, coordinating care across healthcare, behavioral health, and community providers to address complex medical and social needs.

The LCM collaborates with interdisciplinary care teams, community partners, and service providers to promote access to care, improve outcomes, and support member stability and overall well-being.
