East Bay Wrap Program use a high-fidelity model that is honest, family centered, culturally/linguistically responsive and community based.

Wraparound staff work with families utilizing a comprehensive team-based approach.

Our program offers a myriad of services and support including behavioral and mental health assessment and interventions as well as medication and psychiatric support.

Pay/Benefits at a Glance: Pay range is between $25.30 - $32.29 per hour.

What You Will Do: The Care Facilitator is responsible for providing high quality, recovery oriented, strength based, culturally responsive, trauma-informed support services and care coordination to a diverse population of at-risk youth or young adults experiencing mental health symptoms, and their families.

Services include individualized skill-building interventions to reduce symptoms or increase coping skills; transportation and linkage to community resources; and family support, including the facilitation of Child and Family Team Meetings in programs serving youth.

These developmentally appropriate, individualized services occur in residential and community locations including homes, schools, program offices, psychiatric emergency settings, clinics, crisis residential programs, and/or shelters.

The Care Facilitator documents services, including patterns of behavior and incidents, and participates in training and supervisory meetings.

Qualifications: Required B.A.

Degree in counseling, social work, or related field.

Equivalent work and/or volunteer experience in the mental health field may be substituted for each year of required education.At least one (1) year of experience o...
