Registered Nurse

Tender Touch Program | New Jersey Respiratory Associates Are you a Registered Nurse looking to utilize your clinical assessment skills in a unique and impactful way? New Jersey Respiratory Associates (NJRA), in partnership with Tender Touch, is seeking dedicated Registered Nurses to provide specialized PDPM clinical assessments throughout our skilled nursing facility network.

This role combines patient assessment, clinical documentation review, and interdisciplinary collaboration to support skilled nursing facilities in accurately identifying patient conditions and maintaining compliance with Medicare's Patient-Driven Payment Model (PDPM).

As a PDPM Nurse, you will operate on a structured 10-day assessment cycle, conducting evaluations on newly admitted and qualifying residents to ensure accurate clinical documentation and support appropriate PDPM coding and reimbursement.

Responsibilities Conduct comprehensive patient assessments within skilled nursing facilities.

Perform structured evaluations on residents during the facility's 10-day PDPM assessment window.

Review medical histories, physician orders, nursing documentation, and current clinical status.

Identify and document clinical conditions that may impact PDPM classification and reimbursement.

Assess resident needs, treatments, diagnoses, and changes in condition.

Collaborate with facility nursing staff, therapy teams, physicians, and interdisciplinary care teams.

Ensure all assessment findings and recommendations are accurately documented within the designated 10-day assessment cycle.

Support facilities in maintaining accurate clinical documentation and Medicare PDPM compliance.

Educate facility staff regarding clinical documentation requirements and best practices when appropriate.

Maintain confidentiality and comply with all federal, state, and facility regulations.

Qualifications Current Registered Nurse (RN) license in the state of practice.

Minimum of one (1) year of nursing experience.

Strong clinical assessment and documentation skills.

Excellent communication and interpersonal skills.

Ability to work independently and manage multiple facilities and resident caseloads.

Valid driver's license and reliable transportation.

Preferred Qualifications Previous experience in Skilled Nursing Facilities (SNF) or Long-Term Care (LTC).

Experience with PDPM, MDS, Medicare reimbursement, or clinical documentation initiatives.

Experience conducting clinical assessments and interdisciplinary care planning.

Knowledge of regulatory requirements related to post-acute care and skilled nursing facilities.
