Position Summary
The Lead Care Manager (LCM) provides direct services and coordinates care management activities for clients enrolled in the Enhanced Care Management (ECM) Program.
This role includes overseeing assigned cases, facilitating access to health and social services, developing individualized care plans, and collaborating with clients, caregivers, and providers.
The LCM promotes wellness, recovery, resilience, and empowerment while ensuring clients receive appropriate, timely, and cost-effective services.
Key Responsibilities Assessment & Intervention
* Assess client needs across physical health, mental health, SUD, oral health, trauma, social supports, housing, employment, wellness, and referrals
* Conduct intake screenings and ongoing risk assessments
* Provide trauma-informed interventions, coaching, and skill-building
* Deliver services in the client's home or community
* Assist with transportation needs and provide transport when appropriate
