Position Summary

We are seeking a compassionate and organized Discharge Planner / Social Services Assistant to support our Social Services Department and help ensure residents receive the resources, services, and support they need throughout their stay and during discharge planning.

This role works closely with residents, families, community agencies, nursing staff, and healthcare providers to facilitate successful care transitions and promote residents' emotional and psychosocial well-being.

Education & Experience


* High school diploma or equivalent required


* Associate's or Bachelor's degree in Social Work, Human Services, Healthcare Administration, or related field preferred


* Prior healthcare experience preferred


* Skilled nursing, rehabilitation, discharge planning, case management, or social services experience is a plus

Skills & Abilities


* Strong communication and interpersonal skills


* Excellent organizational and time-management abilities


* Ability to work effectively with residents, families, and interdisciplinary teams


* Compassionate and resident-focused approach


* Strong documentation and problem-solving skills


* Ability to maintain confidentiality and professionalism
