Diablo Post Acute is seeking an experienced and compassionate Registered Nurse (RN) - Minimum Data Set (MDS) Coordinator to join our dedicated team!

Full-Time MDS RN

$100,000-$120,000 per year

Responsibilities


* Coordinate and complete MDS assessments in accordance with state and federal regulations.


* Ensure accurate and timely completion of resident assessments and care plans.


* Collaborate with the interdisciplinary team to optimize resident outcomes.


* Maintain compliance with CMS guidelines and documentation requirements.


* Participate in quality improvement initiatives and survey readiness.

Qualifications


* Current RN license in California.


* Previous MDS experience in a skilled nursing facility preferred.


* Strong knowledge of Medicare, Medicaid, and CMS regulations.


* Excellent organizational, communication, and documentation skills.


* Ability to work collaboratively in a fast-paced environment.
