Hayward Gardens Post Acute is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our growing rehabilitation department.

We are hiring Full-Time, Part-Time, and PRN team members who are committed to providing exceptional, resident-centered care in a skilled nursing and post-acute setting.

Working under the direction of a licensed Occupational Therapist, the COTA provides hands-on treatment to help residents regain independence, improve functional abilities, and enhance their quality of life.

Position Summary

The Staff Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist in implementing individualized treatment plans, providing direct resident care, documenting treatment services, and collaborating with the interdisciplinary team to achieve optimal rehabilitation outcomes.

Essential Responsibilities


* Provide occupational therapy treatments as directed by the supervising Occupational Therapist.


* Complete timely, accurate, and compliant clinical documentation in accordance with facility, state, federal, and payer requirements.


* Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.


* Participate in Resident Care Conferences and Rehabilitation meetings as needed.


* Assist the nursing department with training Restorative Aides.


* Communicate regularly with the supervising Occupational Therapist regarding resident progress, treatment goals, physician updates, and discharge planning.


* Provide in-service education and training to facility staff as needed.


* Educate residents, families, and caregivers on treatment goals, home exercise programs, safety techniques, adaptive strategies, and fall prevention.


* Assist with recommendations for adaptive equipment and durable medical equipment (DME) to maximize resident independence with activities of daily living (ADLs).


* Maintain confidentiality of resident and company information.


* Report equipment concerns to ensure safe and effective operation.


* Participate in quality assurance, performance improvement initiatives, and rehabilitation program development.


* Build positive, respectful relationships with residents, families, coworkers, and healthcare professionals.

Qualifications


* Current California COTA license in good standing.
