East Bay Post Acute is seeking a dedicated and dependable Licensed Vocational Nurse (LVN) to join our clinical team on an on-call basis.

As an LVN, you will provide skilled nursing care, support resident wellness, and collaborate with an interdisciplinary team to ensure the highest quality of care in a supportive post-acute environment.
The Opportunity

Position: Licensed Vocational Nurse (LVN) - On-Call
Location: East Bay Post Acute
Pay: $36.00-$40.00 per hour
Schedule: Per Diem / On-Call
Responsibilities


* Provide direct nursing care in accordance with physician orders and facility policies


* Administer medications and treatments accurately and safely


* Monitor residents' conditions and report changes to physicians and nursing leadership


* Complete timely and accurate clinical documentation


* Assist with admissions, transfers, and discharges


* Collaborate with CNAs, nurses, therapists, and physicians to coordinate resident care


* Ensure compliance with infection control, safety, and regulatory requirements


* Support residents and families with compassion and professionalism

Qualifications


* Current California LVN license in good standing


* Current CPR/BLS certification


* Skilled nursing or post-acute experience preferred, but not required


* Strong clinical assessment and communication skills


* Ability to work independently and as part of a multidisciplinary team


* Reliable, flexible, and committed to providing exceptional resident care
