Victorian Post Acute is a beautiful 90-bed skilled nursing facility conveniently located in San Francisco with ample parking.

Our team is dedicated to providing exceptional care with a strong focus on rehabilitation services for both short-term and long-term residents.

Position Details


* Employment Type: Full-Time


* Pay Range: $20.00 - $22.00 per hour

Responsibilities


* Collaborate with residents and their families to establish goals and develop plans that promote independence, self-care, and overall well-being.


* Provide medically related social services to support each resident's physical, mental, and psychosocial needs.


* Assess social, family, psychological, and emotional needs and assist with care plan development.


* Offer emotional support and guidance to residents and families coping with illness, disability, adjustment challenges, grief, and loss.


* Participate in discharge planning and coordinate referrals with appropriate community resources.


* Interview residents, guardians, and sponsors to obtain necessary admission information and documentation.


* Prepare and maintain resident identification records in accordance with facility policies and procedures.


* Maintain accurate records of bed availability.


* Process resident admissions, transfers, and discharges in compliance with established policies and procedures.


* Provide admission packets, including resident rights, privacy notices, and admission agreements.

Requirements


* High school diploma or equivalent required.


* Previous experience in a skilled nursing facility and familiarity with PointClickCare (PCC) preferred, but not required.


* Ability to work effectively in a fast-paced environment while managing multiple priorities.


* Strong communication and interpersonal skills with the ability to build positive relationships with residents, families, and team members.


* Ability to assess social, family, psychological, and emotional needs and contribute to appropriate care planning.
