Key Responsibilities

As a Physical Therapist at Canyon Springs Post Acute, you will play a vital role in helping residents regain strength, mobility, and independence.

Clinical Duties Include:


* Conduct comprehensive evaluations and develop individualized treatment plans based on resident goals and clinical needs


* Provide skilled, evidence-based physical therapy treatments in accordance with the California State Practice Act


* Supervise Physical Therapist Assistants (PTAs) and support Restorative Nursing Aides to ensure continuity of care


* Maintain timely and accurate documentation, including daily treatment notes, progress reports, and recertifications


* Collaborate closely with nursing staff and the interdisciplinary care team; participate in care planning meetings and utilization reviews as needed


* Educate residents and families on mobility safety, home exercise programs, and discharge planning


* Recommend and coordinate durable medical equipment (DME) such as walkers, wheelchairs, and braces to support successful recovery
