Position: Physical Therapist (PT) - Full-Time or Part-Time
Pay Range: $55.00 - $62.00 per hour DOE

Desert Springs Post Acute, soon to be known as Willow Springs Healthcare Center, is seeking a compassionate and dedicated Physical Therapist (PT) to join our growing therapy department.

Position Summary

The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and delivering evidence-based therapy interventions designed to improve mobility, strength, balance, endurance, and overall functional independence.
Essential Duties & Responsibilities


* Evaluate residents and develop individualized treatment plans based on clinical findings and physician orders


* Provide skilled physical therapy services to residents in accordance with established plans of care


* Assess and address mobility limitations, balance deficits, strength impairments, pain management, and fall risk reduction


* Establish realistic, measurable treatment goals and modify plans as needed


* Supervise and collaborate with Physical Therapist Assistants (PTAs) and rehabilitation staff


* Complete timely and accurate documentation in compliance with facility, state, and federal requirements


* Communicate resident progress and recommendations with physicians, nursing staff, families, and the interdisciplinary team


* Educate residents and caregivers on exercises, safety techniques, and discharge recommendations


* Ensure compliance with all professional, ethical, and regulatory standards

Qualifications


* Graduate of an accredited Physical Therapy program


* Current California Physical Therapist license in good standing


* Current CPR certification preferred


* Skilled nursing facility experience preferred but not required


* Strong clinical assessment and treatment planning skills


* Excellent communication and interpersonal abilities


* Ability to work effectively in a team-oriented healthcare environment
