As our Support Coordinator, you’ll help customers build the life skills they need to live independently.

We provide accommodation for customers 18+ who are homeless or at risk of homelessness that have a diagnosed mental health condition.

By supporting our customers, you’ll also help make our service a great place to work, unleashing the potential of colleagues and delivering excellent outcomes.

What you’ll do


* Work with a team of Support Coordinators to deliver a high-quality service that meets Home Group’s KPIs.


* Support customers to build independence and achieve their goals under the guidance of your Senior Client Service Manager, whether this be understanding relationships to addressing substance abuse.


* Assist customers with varied needs including mental health, housing, and complex behaviours and risk.


* Create and update support plans and conduct risk assessments, helping customers with daily tasks like cooking, budgeting, and moving to on into the community.


* Ensure properties are clean, safe, and welcoming, helping reduce voids.


* Enjoy time for meaningful interactions and collaboration, no rushing your chats with customers.

* Experienced in creating person-centred support plans, risk assessments, and reviews.


* Skilled in coordinating and assessing customer referrals.


* Able to work independently, stay calm under pressure, and demonstrate resilience.

* Empathetic and passionate about supporting customers in housing or care settings.
