What You'll Do:


* Help develop, implement, and maintain social services policies and procedures.


* Assess residents' psychosocial, emotional, and medically related social service needs.


* Support residents and families with adjustment to illness, long-term care, discharge planning, and end-of-life concerns.


* Maintain accurate social service documentation, progress notes, and required reports.


* Provide guidance on Medicare, Medicaid, financial assistance, and available community resources.


* Coordinate referrals and services with community agencies and healthcare providers.


* Assist with discharge planning, including arranging equipment, home services, and agency referrals.


* Participate in care planning meetings and interdisciplinary team collaboration.


* Support continuous quality improvement by participating in departmental meetings and policy reviews.


* Build and maintain positive relationships with residents, families, staff, and community partners.

What We're Looking For:


* Education: Must possess, as a minimum, a high school diploma or its equivalent.


* Experience: Prior experience in Social Services or similar role in a healthcare or post-acute setting preferred.


* Compassionate Leader: Demonstrated commitment to resident-centered care and strong interpersonal skills.


* Organized and Detail-Oriented: Excellent organizational skills and the ability to manage documentation and compliance requirements effectively.


* Team Player: Ability to work effectively with multidisciplinary teams and establish positive relationships with residents and families.
