Position Summary

We are seeking a compassionate and organized Social Services Director to oversee and provide social services that support the emotional, psychosocial, and overall well-being of our residents.

This role is responsible for working closely with residents, families, community agencies, and the interdisciplinary care team to ensure residents receive the support and resources they need.
Key Responsibilities


* Assess residents' social, emotional, family, and environmental needs


* Develop and implement individualized social service care plans


* Assist residents in achieving the highest practicable level of independence and well-being


* Provide emotional support and counseling to residents and family members


* Assist residents and families with healthcare decisions and care transitions


* Coordinate discharge planning with community agencies, healthcare providers, and family members


* Arrange ancillary services including optometry, podiatry, dental, and psychiatric appointments


* Connect residents and families with community resources and support services


* Assist with inventory and tracking of resident belongings


* Respond to concerns regarding missing, lost, or stolen belongings


* Maintain accurate and timely documentation of social service interventions


* Participate in care plan meetings and interdisciplinary team discussions


* Develop strong relationships with residents, families, staff, and community partners


* Provide in-service education and training as needed

Qualifications
Education & Experience


* Bachelor's Degree in Social Work or Human Services required


* MSW preferred


* Minimum of two years of supervised social work experience in a healthcare setting


* Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred


* Knowledge of discharge planning, care coordination, and community resources
