Francis Healthcare Center is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis.

As an SLP at St.

Francis Healthcare Center, you'll collaborate with an interdisciplinary team dedicated to delivering exceptional patient-centered care and helping residents achieve their highest level of independence.
Position Responsibilities


* Evaluate residents with speech, language, cognitive, voice, and swallowing disorders.


* Develop and implement individualized treatment plans based on patient needs and physician orders.


* Provide skilled therapy services to improve communication, cognition, and swallowing function.


* Monitor patient progress and adjust treatment plans as appropriate.


* Educate residents, families, and caregivers on treatment goals and home strategies.


* Maintain accurate, timely, and compliant documentation.


* Collaborate with physicians, nursing staff, therapists, and other healthcare professionals to coordinate patient care.


* Ensure compliance with all federal, state, and facility regulations.

Qualifications


* Current Speech-Language Pathologist license in the State of California.


* Certificate of Clinical Competence (CCC-SLP) preferred; strong CF candidates may be considered where applicable.


* Experience in a skilled nursing facility or post-acute rehabilitation setting is preferred.


* Knowledge of dysphagia assessment and treatment.


* Excellent communication, organization, and interpersonal skills.


* Ability to work independently while contributing to a collaborative team environment.
