Key Responsibilities


* Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs.


* Provide skilled physical therapy interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re-education.


* Monitor resident progress and adjust treatment plans to maximize functional outcomes.


* Guide residents through their rehabilitation journey from acute recovery toward safe discharge and community reintegration.


* Collaborate with interdisciplinary team members to coordinate effective, resident-centered care.


* Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.


* Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
