Peninsula Post Acute is a premier short-term rehabilitation and skilled nursing facility dedicated to helping residents recover, regain independence, and return to their highest level of function.

We are seeking a compassionate and motivated Physical Therapist (PT) to join our collaborative in-house rehabilitation team on a PRN basis.

Key Responsibilities


* Evaluate residents and develop individualized physical therapy treatment plans.


* Provide evidence-based physical therapy services to improve mobility, strength, balance, endurance, and overall functional independence.


* Monitor resident progress and modify treatment plans as appropriate to achieve optimal outcomes.


* Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.


* Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to coordinate high-quality patient care.


* Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.


* Participate in interdisciplinary care conferences and discharge planning.


* Educate residents and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques.

Qualifications


* Current California Physical Therapist (PT) license in good standing.


* Strong communication, clinical reasoning, and teamwork skills.


* Passion for delivering high-quality rehabilitation services.
