Victorian Post Acute is a 90-bed, rehab-focused skilled nursing facility dedicated to providing exceptional care for both short-term rehabilitation and long-term care residents.

We are seeking a compassionate and motivated PRN Speech-Language Pathologist (SLP) to join our collaborative in-house therapy team.

Key Responsibilities


* Evaluate residents and develop individualized speech-language pathology treatment plans.


* Provide skilled therapy services for speech, language, cognitive-communication, voice, and swallowing disorders.


* Perform clinical swallowing evaluations and implement appropriate dysphagia management strategies.


* Collaborate with physicians, nursing staff, dietitians, rehabilitation professionals, residents, and families to achieve optimal outcomes.


* Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.


* Participate in interdisciplinary care conferences and discharge planning.


* Educate residents, caregivers, and staff on communication strategies, swallowing precautions, and home programs.


* Ensure compliance with all federal, state, and facility documentation and regulatory requirements.
