Position Summary:
NeighborHealth is seeking a PACE Physician to provide comprehensive primary care to older adults with complex medical and functional needs enrolled in our nationally recognized Program of All-Inclusive Care for the Elderly (PACE).
Physicians benefit from a predictable schedule, interdisciplinary team-based care, and the opportunity to deliver primary care services across PACE clinics, at home, assisted living, and nursing facilities.
Salary range: $229,200 to $271,200 based on experience.
Physicians may also be eligible for federal and state loan repayment programs.

What You'll Do:
* Monday through Friday schedule, 40 hours per week, 8 hours per day.
* Provide primary care services to frail participants enrolled in the PACE program
* Care delivered across clinic, home, assisted living, and nursing facilities
* Care in all settings is provided in concert with an interdisciplinary care teams of health professionals in a progressive health care organization.
* EPIC electronic medical record
* Established, nationally recognized PACE program with 35+ years of history

What You'll Bring:
* Board Certified or Board Eligible in Internal Medicine or Family Medicine
* Geriatrics certification preferred but not required
