About the Role
The PSH Housing Coordinator provides housing case management and supportive services to residents of Compass Health's Permanent Supportive Housing (PSH) program.
This position helps individuals exiting homelessness, hospitals, jails, and residential treatment maintain stable housing while promoting recovery, independence, and overall wellness.
Working collaboratively with residents, clinical providers, property management, community agencies, and landlords, the PSH Housing Coordinator assists clients throughout the housing process-from eligibility and lease-up through long-term housing stability-using a Housing First, trauma-informed, and person-centered approach.
Key Responsibilities Housing Coordination & Case Management
* Assist residents receiving Shelter Plus Care, VASH, ALTSA, and other housing subsidies in obtaining and maintaining permanent supportive housing.
* Guide residents through housing eligibility, lease-up, and documentation requirements, including obtaining identification, income verification, disability documentation, and homelessness verification.
* Support new residents in establishing daily routines, meeting basic needs, and successfully transitioning into housing.
* Coordinate care planning with VA, ALTSA, Compass Health clinicians, and other treatment providers.
* Maintain collaborative relationships with property management to resolve tenancy concerns, lease issues, and housing barriers.
* Attend housing program briefings, lease violation meetings, and housing termination appeals as needed.
* Provide housing support for individuals transitioning from Western State Hospital and other institutional settings.
* Meet required client contact standards, including home visits and community-based outreach.
* Provide crisis intervention, advocacy, outreach, and case management services in homes, shelters, hospitals, jails, community agencies, and other community settings.
* Support residents in developing independent living skills and maintaining housing stability.
* Collaborate with community providers to coordinate services that promote long-term recovery and successful community living.
* Maintain accurate documentation, case notes, incident reports, home visit reports, and required internal and external reports.
* Participate in the after-hours on-call rotation.
* Identify opportunities to improve and expand Permanent Supportive Housing services.
* Participate in regularly scheduled supervision, team meetings, and required trainings.
