The LPN Care Coordinator position serves as a liaison, promoting coordination, communication and collaboration among Brookdale teams, residents, families and healthcare providers within the community.

This position works closely with primary and specialty care providers establishing care-plans.

The LPN Care Coordinator will also serve as the resident advocate

This position will be responsible for overseeing 3 Brookdale communities

Base pay in range will be determined by applicant's skills and experience.

Temporary associates are not benefits eligible but may participate in the company's 401(k) program.

* Graduate from an accredited LPN program and has successfully passed the state licensing test.


* Knowledge of value-based care models is preferred.


* Experience working in SNF and HH environments is a preferred.


* Previous Care Management, Case Management, ...
