Lone Tree Post Acute is seeking a compassionate and skilled Occupational Therapist (OT) to join our in-house rehabilitation team.

What You'll Do


* Evaluate residents and develop individualized, evidence-based treatment plans


* Provide skilled occupational therapy services focused on functional outcomes and independence


* Supervise and mentor COTAs and therapy students


* Participate in care planning meetings and interdisciplinary collaboration


* Maintain accurate clinical documentation and ensure regulatory compliance


* Support safe and successful discharges through caregiver education and discharge planning

Qualifications


* Current California Occupational Therapist (OT) license


* Strong clinical, communication, and leadership skills


* Commitment to resident-centered care and positive outcomes
