Pleasant Hill Post Acute is seeking a compassionate and skilled Physical Therapist (PT) to join our in-house rehabilitation team.

Responsibilities


* Evaluate residents and develop individualized treatment plans


* Provide skilled physical therapy services to improve mobility, strength, and independence


* Supervise PTAs and collaborate with the interdisciplinary care team


* Participate in care planning and discharge preparation


* Educate residents, families, and caregivers


* Maintain accurate and timely clinical documentation

Qualifications


* Current California Physical Therapist (PT) license


* Strong clinical and communication skills


* Commitment to resident-centered care


* Ability to work effectively within a multidisciplinary team
