Position Summary

Medical Hill Healthcare is seeking an experienced and detail-oriented Quality Assurance Nurse (LVN) to lead and support quality improvement initiatives throughout the facility.

This position is responsible for planning, coordinating, and overseeing Quality Assurance and Performance Improvement (QAPI) activities to ensure the highest standards of resident care, regulatory compliance, and clinical excellence.

The QA Nurse works collaboratively with facility leadership and the interdisciplinary team to monitor clinical outcomes, identify opportunities for improvement, and implement strategies that enhance quality and resident satisfaction.

Essential Duties and Responsibilities

Quality Assurance & Performance Improvement (QAPI)


* Develop, implement, and maintain facility-wide quality assurance and performance improvement programs.


* Monitor clinical systems and processes to ensure compliance with company standards and regulatory requirements.


* Participate in and support facility committees related to quality improvement, risk management, and resident care.


* Evaluate resident care practices and promote resident-centered care and choice.

Data Analysis & Reporting


* Collect, analyze, and interpret clinical quality indicators and performance data.


* Compare facility outcomes against established benchmarks and identify trends requiring intervention.


* Prepare reports, presentations, and recommendations for the QAPI Committee and facility leadership.


* Monitor ongoing performance improvement plans and track outcomes.

Infection Prevention & Control


* Assist with oversight of the Infection Prevention and Control Program.


* Conduct surveillance, tracking, investigation, and reporting of communicable diseases and infection-related incidents.


* Monitor compliance with infection control policies and regulatory guidelines.


* Support staff education and corrective action initiatives related to infection prevention.

Regulatory Compliance & Risk Management


* Participate in preparation for State and Federal surveys and assist with corrective action plans related to survey findings.


* Investigate adverse events, incidents, and quality concerns and develop action plans to prevent recurrence.


* Ensure timely documentation and follow-up of quality-related issues.


* Support ongoing compliance with federal, state, and facility regulations.

Staff Education & Collaboration


* Collaborate with the Interdisciplinary Team (IDT) to identify educational needs based on quality data and clinical trends.


* Assist in developing and delivering staff training, in-services, and competency programs.


* Promote a culture of continuous quality improvement and accountability throughout the facility.
