Physical Therapist Responsibilities


* Evaluate residents and develop individualized treatment plans


* Deliver evidence-based physical therapy services in a skilled nursing and post-acute setting


* Establish functional goals and help patients regain mobility and independence


* Complete evaluations, progress notes, discharge summaries, and required documentation


* Collaborate with physicians, nursing staff, families, and interdisciplinary team members


* Educate patients and caregivers on treatment plans and home exercise programs


* Contribute to exceptional patient outcomes and satisfaction

Qualifications


* Active California Physical Therapist (PT) license or eligibility to obtain licensure


* New graduates encouraged to apply


* Skilled nursing, rehabilitation, or post-acute experience preferred but not required


* Strong clinical reasoning and communication skills


* Excellent documentation and time-management abilities
