Position Summary

We are seeking a licensed Occupational Therapist (OTR) to provide high-quality, patient-centered therapy services.

This role includes evaluation, treatment planning, team collaboration, and supervision of therapy staff, all focused on improving residents' independence and quality of life.
Key Responsibilities


* Evaluate residents within required timeframes and develop individualized treatment plans


* Provide therapy services per physician orders and established goals


* Document evaluations, treatments, and progress notes in compliance with regulations


* Supervise COTAs, aides, and students in accordance with state practice act


* Collaborate with the interdisciplinary team on care planning and patient progress


* Participate in care conferences, utilization reviews, and rehab meetings


* Educate residents, families, and caregivers on therapy programs and discharge plans


* Assist with durable medical equipment (DME) recommendations


* Provide staff training and participate in in-services


* Ensure compliance with all state, federal, and payer requirements

Qualifications


* Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred)


* Active South Carolina Occupational Therapist (OTR) license in good standing


* Experience in skilled nursing or rehab setting preferred


* Familiarity with PointClickCare and Casamba preferred


* Strong clinical, documentation, and leadership skills
