π Responsibilities:
πΉ Evaluate residents with speech, language, cognitive, communication, and swallowing disorders
πΉ Develop individualized treatment plans based on resident needs and goals
πΉ Provide skilled speech therapy services in accordance with physician orders and established care plans
πΉ Educate residents, families, and caregivers on therapy goals and treatment techniques
πΉ Collaborate with interdisciplinary team members to ensure optimal patient outcomes
πΉ Complete timely and accurate documentation in compliance with regulatory requirements
πΉ Participate in care plan meetings and discharge planning as needed
πΉ Maintain compliance with all federal, state, and facility guidelines
π Qualifications:
βοΈ Master's Degree in Speech-Language Pathology
βοΈ Current Speech-Language Pathologist License (State Requirements)
βοΈ Certificate of Clinical Competence (CCC-SLP) preferred
βοΈ Experience in skilled nursing or rehabilitation settings preferred, but not required
βοΈ Strong communication and interpersonal skills
βοΈ Passion for helping others achieve their highest level of independence
