RN Care Manager - Transitional Care Clinic
Vancouver Clinic is hiring a full-time RN Care Manager ( 36 hours /week) to support our Transitional Care Clinic.
In this role, the RN Care Manager will coordinate patient care, helping ensure a seamless, supportive experience for patients and their families.
This position is primarily focused on care coordination, patient and family education, psychosocial support, and improving quality of life through compassionate, collaborative care, with additional responsibilities that may include triage, administering treatments, and assisting with minor procedures.

Schedule: Monday-Friday, 8:00 a.m.
to 4:00 p.m.; schedule may vary based on department needs.
H iring range is generally between $ 46.10 /hour -$ 58.73/hour and placement in the range is dependent on an evaluation of experience.
Requirements:
* Active, unencumbered Washington or Multi-State RN license require d .
* BSN or MSN/MN degree in nursing (BSN or MSN/MN) strongly preferred
* Minimum 2 years clinical nursing experience required with relevant experience in one or more of the following healthcare areas preferred: Coordination of community resources, Care management of diverse patient populations, Ambulatory Care
* Case management certification strongly preferred.
Vancouver Clinic provides care across a wide range of medical decisions.
This includes care and opinions on vaccinations, reproductive health, end-of-life decision-making, and gender affirming treatment.

Pay Range:
$45.20 - $67.80
