Midtown Oaks Post Acute is looking for an organized, proactive, and customer-service-oriented Ward Clerk to join our team.

This is a critical role for someone who thrives in a fast-paced healthcare environment and enjoys coordinating care, solving problems, and ensuring residents receive the services they need.

We're seeking an experienced professional who can confidently manage transportation scheduling, coordinate specialty appointments, process referrals, and serve as a key communication link between residents, families, healthcare providers, and our interdisciplinary team.

What You'll Do


* Schedule and coordinate resident transportation for medical appointments, treatments, dialysis, and specialty visits


* Arrange and track referrals for specialty physicians and outside healthcare providers


* Communicate with physician offices, transportation vendors, hospitals, clinics, and family members as needed


* Ensure appointments, authorizations, referrals, and supporting documentation are completed accurately and timely


* Maintain appointment calendars and track upcoming visits and follow-up care


* Assist with medical records, chart preparation, and administrative support functions


* Coordinate communication between nursing, therapy, social services, physicians, and outside providers


* Answer phones, greet visitors, and provide exceptional customer service


* Support the interdisciplinary team with various clerical and administrative tasks

What We're Looking For

Required


* Previous experience in a healthcare administrative, ward clerk, medical receptionist, medical records, or similar role


* Experience scheduling medical transportation and coordinating appointments


* Strong organizational and multitasking skills


* Excellent communication and customer service abilities


* Proficiency with computers, electronic medical records, and Microsoft Office applications

Preferred


* Skilled nursing facility, post-acute, hospital, or healthcare experience


* Experience handling specialty referrals, insurance authorizations, or appointment coordination


* Knowledge of healthcare terminology and medical office workflows
