We're seeking a dedicated Physical Therapist to join our outstanding rehabilitation team on a Part-Time or Per Diem basis.

What You'll Do


* Evaluate residents and develop individualized treatment plans


* Provide skilled physical therapy services to post-acute and long-term care residents


* Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families


* Monitor patient progress and modify treatment plans as needed


* Maintain accurate and timely documentation


* Educate residents and caregivers on exercises, safety, mobility, and discharge recommendations


* Help residents achieve their highest level of independence and quality of life

Qualifications
Required:


* Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program


* Current California Physical Therapist license in good standing


* Strong clinical assessment and treatment skills


* Excellent communication and teamwork abilities

Preferred:


* Experience in Skilled Nursing, Post-Acute, Rehabilitation, or Long-Term Care settings


* Knowledge of Medicare documentation and reimbursement guidelines


* Experience treating orthopedic, neurological, and medically complex patients
