Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and functional independence.

You will collaborate with physicians, families, and interdisciplinary team members to deliver high-quality, patient-centered care.
Key Responsibilities


* Perform patient evaluations and develop individualized treatment plans


* Provide therapy to improve strength, balance, mobility, and function


* Monitor patient progress and adjust treatment plans as needed


* Maintain accurate and compliant clinical documentation


* Communicate effectively with physicians, residents, families, and care team members


* Participate in care planning and discharge planning


* Provide education and guidance to residents and staff

Qualifications


* Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred)


* Active, unencumbered Physical Therapist license in South Carolina (required)


* Must maintain all continuing education and remain in good standing with the State Board


* Experience in a skilled nursing or rehab setting preferred


* Familiarity with Net Health and PointClickCare (PCC) preferred


* Strong clinical, communication, and organizational skills


* CPR certification (if required by licensure)
